Change to Form 8822-B (Rev. January 2012) -- 28-FEB-2013

There is a change to Form 8822-B (Rev. January 2012). In the "Where To File" section on page 1, if your old business address was in Florida, the new filing address is:

Internal Revenue Service Center
Cincinnati, OH 45999-0023

This change will be included in the next revision of the Form 8822-B.
