Form 8821, Tax Information Authorization - The new fax number for Philadelphia may not yet be operational -- 09-Nov-2012
The following is an update to Form 8821, Tax Information Authorization (Rev. October 2012)
At the bottom of page 3 there is a chart titled: Where To File Chart. The third column fax number for Philadelphia, 855-772-3156, may not yet be operational. The previous fax number, 267-941-1017, can still be used until the new fax number is operational. Forms 8821 faxed from outside the United States should continue to use the fax number 267-941-1017.
