Corrections to Publication 1141 (Revenue Procedure 2011-62) -- 16-MAY-2012
If you downloaded Publication 1141 (Revenue Procedure 2011-62), General Rules and Specifications for Substitute Forms W-2 and W-3, before January 18, 2012, please note the following corrections.
On page 24, Exhibit B, Form W-3 (Red-Ink), 2011, note that the dimensional left-right arrows in Box b, Kind of Payer, should be placed as follows, beginning from left to right:
On the bottom row of checkboxes:
- The left-right arrow should be 1.2" originating from the form's left vertical line and should end on the left vertical line of the CT-1 checkbox.
- The two left-right arrows (.36") should be centered directly between the CT-1 and Hshld. emp. checkboxes.
- There should be no left-right arrows to the right of the Medicare govt. emp. checkbox.
A revised version of the Publication 1141 will be reposted at a later date.
