To apply for QI status, please complete the QI Status Application Form 14345 and Form SS-4.  The effective date of January 1, 2011 will be restricted to those QI applicants only whose application for QI status is received at the following address before the deadline of November 25, 2011:

The applications for QI status received after this deadline will be processed effective January 1, 2012.

Further, the deadline for assigning EINs effective January 1, 2011 is December 23, 2011.  Signed copies of the QI Agreements received after this deadline will be assigned EINs effective January 1, 2012.
