Problem With Name and Identifying Number

Your tax return for {tax year} shows a different last name and identifying number from the information shown on our records or provided by the Social Security Administration (SSA).

This can occur if you filed your tax return using a maiden, married, or professional name that the Internal Revenue Service (IRS) and/or SSA does not have on record. It can happen if a name is misspelled or the Identifying Number is transposed.

The Identifying Number may be a Social Security Number (SSN) or an IRS Individual Taxpayer Identification (ITIN) assigned if you do not qualify for an SSN.

If you have an SSN, please compare the name and Identifying Number that appear at the top of this notice with the SSN on your most recent social security card.

If there is a difference noted (misspelling, transposed numbers, etc.), and you did not change your name:

  • Please explain the difference by completing the information on the last page of this notice.
  • Provide COPIES of documentation (marriage certificates, court records, etc. that support your name change).
  • If the Taxpayer Identifying Number (TIN) shown above does not match the number on your current social security card, please enter the correct number in space provided the last page of this notice.

If your name is different and it does not agree with the name shown on your most recent Social Security Card:

  • Notify SSA of the change. If you have already done this, do not notify them again.
  • If you do not notify SSA of the change, send us a COPY of your marriage certificate or court records clearly showing a legal name change.
  • We will update our records using information provided.

If there is no difference:

  • Contact SSA at their toll-free number {phone #} to ensure their records are correct.
