If you cannot get an SSN because you do not qualify:

  • Please get a Form W-7, Application for IRS Individual Taxpayer Identification Number, from your local IRS office or call {toll-free #}.
  • AIternative: The W-7 is available on the IRS Web page at www.irs.gov
  • Please complete the Form W-7. Include all supporting documentation to substantiate the application. Also, include any documentation you received from SSA rejecting your request for an SSN.
  • Mail the Form W-7 and supporting documentation requested to the Internal Revenue Service, {appropriate address}, Attn: ITIN Unit.
  • If we approve your Form W-7 application, you will receive a permanent IRS Individual Taxpayer Identification Number (ITIN).
  • Keep the ITIN card and use the ITIN for tax purposes only. We will return all your original documents.
  • If you owe no other taxes or legal debts we are required to collect and are due a refund, you will receive your refund in approximately four weeks after we receive your reply.

Complete and return your reply in the enclosed envelope by {due date} .

Note: Contacting the SSA with all name changes helps to ensure your Social Security payments are applied correctly.
